Chances are, EZSurvey is configured to use the wrong directory. On the main window, go to Email Advanced email options... and remove the "Database working directory" entry. EZSurvey may also be checking for a specific subject line. You can change that in Email Advanced email options... by changing the box labeled Look for the subject.
